Who we Support

We provide supported living services for young people and adults aged 16–25 who are transitioning from care, as well as adults with learning disabilities, autism, acquired brain injuries, and complex needs. Our focus is on creating safe, homely environments where individuals can build independence, develop life skills, and receive the right level of support tailored to their needs.

Our referral process

We’ve made our referral process simple and responsive to ensure the right support is in place as quickly as possible.

Step 1 – Submit Referral
Referrals can be made directly through our online form, by email, or by phone. We acknowledge all referrals within 24 hours.

Step 2 – Assessment & Placement Matching
Our team carries out a thorough needs assessment and risk screening to ensure we can provide the right level of support. We then match the individual to the most suitable placement.

Step 3 – Transition & Onboarding
Once a placement is agreed, we create a personalised transition plan, working closely with families and professionals to make the move as smooth as possible.
